E and A are contained in an anagram of accent. Yes, crosswords use A-G to signify notes.

I'd made a mistake with two clues intersecting with the second one: Expand direction on Elgar composition (7) which I had as ENIGMAS but I now realise is ENLARGE; and Dine more extravagantly as leader (7) which I had as SUPREME but I now realise is SUPREMO. The final E and O fit FALLOPIAN TUBE, as does the L with AROUSAL and I can now see that Island nation has five, right behind urn (7) is SAMOVAR.

 



Aim for treatment is fibroma.

I.e. initially benign => B + AIM + FOR? My knowledge of cancer treatment vocabulary doesn't extend beyond chemotherapy and radiotherapy. :sick:

 



Think the sarcastic one could be caustic soda.

The fibroma one is anagram ( treatment of ) aim for including first initial of benign.
Caustic soda is caustic meaning sarcastic ,like this is so and DA is District Attorney. Caustic soda is a sort of soap.
